Looking for your COVID-19 vaccination record?

The California Department of Public Health has created an online portal where you can access your vaccination information. The site will give you a digital copy of your vaccine record, which you can also print. Go to

If you notice an error in the state’s record, please contact the entity that gave you the vaccine to get the information corrected.

Prevent Household Transmission of COVID-19
One of the most important ways to slow the spread of COVID-19 is to protect others in the same household from contracting the virus

Have you had:

  • Symptoms of COVID-19?
  • Close contact (been within 6 feet) of an infected person for 15 minutes?
  • A COVID-19 test (but are still waiting for results), or
  • A diagnosis of COVID-19?
If yes to any of these, then IMMEDIATELY:
1. Call your medical provider

If You Have Symptoms Of Covid-19, But Don’t Have Health Insurance

If you or someone you know is experiencing symptoms of COVID-19, it’s important to know your insurance options. If you do have insurance, you should call  your regular doctor or clinic. If not, this flyer from San Mateo County Health describes your options and offers general COVID-19 related guidelines. 

This flyer is available in several languages including, English, Spanish, Chinese, Tagalog, Samoan, Tongan, and Russian.

San Mateo County Health has partnered with a company called Qualtrics on secure web-based surveys to aid in the case investigation and contact tracing processes.

If you test positive for COVID-19, you will receive a health assessment survey developed in Qualtrics that asks about your health and who you have been in close contact with.
