CalAIM Enhanced Care Management & Partners for Independence Program
General information
The CalAIM Enhanced Care Management (ECM) and Partners for Independence (PFI) programs provided social and health care management to people aged 21 or older who are eligible for Medi-Cal and are Health Plan of San Mateo (HPSM) members.
Both CalAIM ECM and PFI programs are voluntary programs providing intensive care management to older adults and/or people with disabilities with medical and psychosocial needs. Care management services are provided to help reduce adverse health and psychosocial outcomes.
CalAIM ECM and PFI goals include:
- Preserving client self-determination to remain home safely for as long as possible,
- Promoting positive client outcomes through intensive case management,
- Providing coordinated care using a collaborative approach with service providers and
- Preventing or delaying out-of-home care.
