Whole Person Care

San Mateo County Health started the Whole Person Care pilot in 2016 in response to Medi-Cal 2020. Whole Person Care (WPC) was initiated with a funding stream designed to enhance, evolve, and transform service delivery within the health care system. It is a coordinated approach where we proactively communicate across divisions and departments, supporting the most complex and vulnerable clients with agility, efficiency, and perseverance.

Whole Person Care is grounded in the County Health’s philosophy of caring for the “whole person.” WPC seeks to address the physical, mental health, substance use, and social needs of eligible clients. To do this, we are working to strengthen collaboration across divisions and with our county and community partners. Our goal is to provide coordinated care for the most vulnerable clients with complex needs. The WPC initiative has funded the expansion of existing services, as well as new services and programs that enhance our ability to serve the whole person.
