Looking for your COVID-19 vaccination record?

The California Department of Public Health has created an online portal where you can access your vaccination information. The site will give you a digital copy of your vaccine record, which you can also print. Go to myvaccinerecord.cdph.ca.gov.

If you notice an error in the state’s record, please contact the entity that gave you the vaccine to get the information corrected.

Legislation, Advocacy & Outreach Committee: Every 2nd Monday of the month, except for August & December

Americans with Disabilities Act (ADA) Compliance Committee: Every 1st Thursday of the month, except for August & December

Special Events: Every 3rd Tuesday of the month, except for August & December

Youth & Family Committee: Every 3rd Monday of the month, except for August & December

Accessible Transportation: Every 4th Monday of the month, except for August & December
