Below are Certified Unified Program Agency (CUPA) forms and permits you may need for your business. 

How to Apply

New! Apply Online with MyEHS
You can now apply for CUPA permits, pay invoices, and manage your permits online using MyEHS.

Payments cannot be made to or accepted by inspectors. Payments must be made in person at the Environmental Health Services office, by mail, by phone at (650) 372-6200, or online through the MyEHS Portal.

If you’re unable to apply online with MyEHS, you can download and complete your application at smchealth.org/myehs (see Resources & PDF Applications). Under this section you will find:

- New Facility Permit Registration Form
- Permit Application to Construct/Replace/Modify Underground Storage Tank System
- Underground Storage Tank System Closure Permit Application
– Hazardous Materials Business Plan Program Exemption Application for Temporary Emergency Generator Deployment. Please note: a CUPA Permit is not required if the temporary emergency generator deployment is less than 30 days.
