Information for Teen Patients 12-17

Teen patients ages 12–17 years old are given full access to their electronic health record via MyChart. Teen patients can decide what level of proxy access their parent or guardian has to their medical record.

What types of proxy access can a teen give their parent/guardian?

  1. “Parent Accessing Teen” proxy access includes administrative information but NO clinical information can be accessed.
  2. “Parent Accessing Teen Expanded” proxy access includes: administrative information, appointment scheduling, and clinical features such as visit notes, diagnosis, medications, lab results, upcoming appointments, and messages to your care team.

Can a teen change their mind about giving someone access to their health information in MyChart?

Yes. A teen can always change their mind in the future about the level of access they grant to a parent/guardian.

What rights do teens have to confidential medical visits and health information?

In California, the law allows teens to get services related to reproductive health, substance use, and mental health treatment without consent of a parent or guardian. A teen’s medical care team cannot share confidential sensitive information with a parent/guardian without their prior written consent or authorization. (If a teen is being harmed or plans on harming themselves, the care team is legally required to get outside help.)

How can a teen schedule a confidential visit with their doctor or nurse?

To schedule a confidential visit, call the clinic and schedule the appointment directly. Let them know you want the appointment to be confidential.
